If you are a physician who refers patients to University Hospital, Children's Hospital, Columbia Regional Hospital, Ellis Fischel Cancer Center or our clinics, you may request access to view the medical record for your referred patient.

If you work with a referring physician, either as a care provider or as support staff, you may request access to view the electronic medical record for patients referred by your physician to University Hospital, Children's Hospital, Columbia Regional Hospital, Ellis Fischel Cancer Center or our clinics.

To request access, you must complete both the "Access and Confidentiality Agreement" and the "Referring Physician's Online Access Form".

Your Sponsoring Physician MUST be assigned access prior to care providers and/or support staff receiving access.

What happens next?

After University of Missouri Health Care Medical Records receives your online registration, you will receive your access information via a confirmation email within 48 business hours. Then you may access your patients' electronic medical records, anytime, anywhere.
